We support NHS organisations, local authorities, care providers and other system partners where a situation, pathway or service needs independent challenge, practical problem solving or a different perspective.

This may include complex cases where discharge has stalled, community options appear limited, restrictive practice has become entrenched, relationships between organisations have become difficult, or existing support arrangements are no longer working.

We also work with care providers and leadership teams who want to better understand the wider health and social care system, strengthen their approach to commissioning and develop services that respond to what people and local systems actually need.

Our work draws on experience across learning disability and autism, clinical practice, commissioning, service development and complex community support.

This can include:

• Independent complex case reviews and recommendations
• Discharge and community pathway planning
• Reviewing barriers to community living
• Support model and transition planning
• Restrictive practice and human rights review
• Provider and system problem solving
• Commissioning insight and strategic review
• Workshops, facilitated sessions and leadership development

The aim is not simply to produce another report. It is to understand why progress has become difficult, identify what needs to change and help create a way forward that can realistically be implemented.

Strategic Case Resolution and Brokerage

Everyone agrees something needs to change, but the system still can't find a way forward.

Some people remain in inappropriate places despite multiple reviews, meetings, recommendations and plans. They may be experiencing a delayed discharge, living in an inappropriate or overly restrictive environment, placed far from home, experiencing repeated placement breakdown, or receiving a high-cost package of care that is no longer delivering the outcomes intended.

We provide independent strategic review of what is stopping progress, rather than another assessment of the person.

Working across a small cluster of complex situations, we examine what has already been recommended, what has and hasn't happened since, and where the real barriers now sit. This might include commissioning arrangements, housing, provider availability, funding, risk assumptions, support model design, relationships between organisations or a lack of clarity about who needs to do what next.

Understand. Unblock. Advise.

For each person, we develop a clear Recommendation Plan, identifying the barriers to progress, practical options for moving forward and the decisions or actions required. Where appropriate, we can identify alternative models of support, relevant market options and potential providers for the commissioning team to explore.

We don't take over statutory decision-making, commissioning or procurement. Our role is to bring independent scrutiny, challenge and practical expertise to cases where existing processes have not resulted in meaningful progress.

Looking at systems

When several people within the same system are stuck, patterns begin to emerge.

By reviewing cases as a group, we can identify recurring issues such as gaps in the local provider market, housing shortages, commissioning barriers, unrealistic specifications, delays in decision-making, risk aversion or problems translating agreed recommendations into action.

Alongside individual Recommendation Plans, leaders receive an analysis identifying the systemic barriers affecting people's progress and practical recommendations for addressing them.

The result is not simply a clearer way forward for the people being reviewed. It gives organisations intelligence about why people are becoming stuck in the first place and what could prevent it happening again.

Who is this for?

Independent Case Reviews can support Local Authorities, Integrated Care Boards, NHS Foundation Trusts and other system partners with a group of people experiencing delayed discharge, inappropriate or high-cost arrangements, out-of-area support, repeated community instability or situations where there is agreement that change is needed, but no clear route to achieving it.

We will help you understand what is preventing progress and what the system could do differently.

Have people in your system who simply aren't moving?

Bring us the cases that keep coming back to the same meeting.

We will help you understand where they are stuck, what needs to change and what it will take to move forward.

Strategic Advisory for Providers

growing sustainably, and in the right way

Growth in social care is not simply about "finding more referrals".

Providers are making decisions in a system where commissioning priorities are changing, funding pressures are significant, local needs vary considerably and the services that commissioners need are not always the services already available.

The challenge is often knowing where to focus.

Which opportunities genuinely fit your organisation? Where are the gaps in local provision? What are commissioners struggling to solve? Which services should you develop, and which opportunities should you be prepared to walk away from?

Ashleigh Fox Advisory works with care provider CEOs, senior leadership teams and business development teams who want an experienced external perspective on those questions.

Understanding the system you are growing into

Health and social care can be difficult to navigate from the provider side.

Commissioning structures, funding routes, NHS and local authority responsibilities, market-shaping priorities and decision-making processes can all influence whether a service succeeds long before the first person is supported.

We can help leadership teams understand the wider system around their organisation, including how commissioners think about need, risk, quality, value and sustainability, and how providers can engage with those conversations more effectively.

This is not about learning how to “sell” to commissioners - it is about understanding the problems local systems are trying to solve and considering where your organisation can genuinely contribute.

Turning good ideas into services that are needed

Sometimes an organisation has a strong idea but needs to test whether it responds to a genuine system need.  Sometimes the opportunity is obvious, but the model needs further thought.

And sometimes the most useful external advice is to challenge an opportunity before significant time, money and organisational energy are committed to it.

We can work alongside leadership teams to explore new service models, test assumptions, consider commissioning and operational realities and think through what would need to be true for an idea to work well for the people it is intended to support.

Building relationships and trust

Sustainable growth in social care depends on trust.  Commissioners and professionals need confidence that a provider understands the people they support, can work constructively when situations become difficult and will be honest about what it can and cannot deliver.

We can help organisations think about how they communicate their offer, develop meaningful relationships across local systems and build a reputation around the problems they are genuinely good at solving.

The aim is to become clearer about where your organisation adds value and make it easier for the right people to understand that.

Independent challenge for leadership teams

Ashleigh Fox Advisory can provide regular independent thinking space for CEOs and senior teams, bringing together an understanding of commissioning, complex care, service development, clinical practice and the realities of delivering support.

That might mean challenging a growth strategy, exploring a new market, thinking through a difficult commissioning relationship, reviewing a proposed service model or simply providing an external perspective before an important decision is made.

Support can be provided through individual strategic sessions, defined projects or an ongoing advisory arrangement.

growth with purpose

The question is not simply: how can we grow?

It is: where should we grow, what are we genuinely good at, what do local systems need, and will that growth ultimately create better support for people?

The strongest organisations are not necessarily those that pursue every opportunity.

They are the ones that understand where they can make the greatest contribution and build sustainably around it.
